36, Okhla · advised laparotomy for an 'ovarian mass'

What she came in with

She arrived with a scan report describing a 7 cm complex adnexal mass and a recommendation for open surgery with possible oophorectomy, scheduled for the following week.

What the workup showed

Repeat transvaginal ultrasound with Doppler showed a mass with ground-glass echogenicity, no papillary projections, no internal vascularity — the classic appearance of an endometrioma, not a neoplasm. CA-125 was mildly raised at 58 U/ml, which endometriomas commonly cause. She had severe dysmenorrhoea, which had never been asked about.

What we did

Laparoscopy rather than laparotomy, with AMH measured beforehand, ovarian tissue-sparing cystectomy, and excision of associated peritoneal disease in the same sitting.

Outcome

Histology confirmed an endometrioma. Her ovary was preserved, she went home the next day, and her period pain resolved.

The point of this case

A mildly raised CA-125 with an endometrioma is expected and frightens people unnecessarily. Twenty minutes of history-taking and a careful scan changed an open operation into a day-case laparoscopy.
