Case study: A placenta praevia found before it bled

What she came in with

A routine anomaly scan at 19 weeks in an otherwise unremarkable pregnancy. She had had one previous caesarean section.

What the workup showed

The placenta was anterior and covering the internal os. With a previous caesarean this raises the question of placenta accreta spectrum — placental tissue invading the old scar — which is the single most dangerous obstetric complication of a scarred uterus.

What we did

Serial scans at 28 and 32 weeks confirmed persistent praevia, with colour Doppler and a dedicated accreta assessment showing loss of the clear zone and bridging vessels. Delivery was planned at 36 weeks in a hospital setting with blood products cross-matched and a multidisciplinary team briefed.

Outcome

An elective caesarean at 36 weeks with the team assembled and blood available. Accreta was confirmed and managed in a controlled setting rather than as an emergency haemorrhage.

The difference between a planned accreta delivery and an unplanned one is measured in litres of blood. A 19-week scan bought seventeen weeks of preparation.
