Case study: Three years of trying, and nobody had checked the tubes

What she came in with
Three years of trying, several courses of ovulation-inducing tablets prescribed by a local practitioner, and no scan monitoring at any point. She had never had a tubal assessment, and her husband had never had a semen analysis.
What the workup showed
Regular ovulatory cycles on tracking. HSG showed both tubes patent. Semen analysis showed a total motile count at the lower end of normal. Nothing was badly wrong with either of them — which is a diagnosis, not the absence of one.
What we did
Rather than escalating to IVF, three cycles of letrozole with scan-monitored follicular tracking and timed IUI with a washed, concentrated sample. Endometrial thickness was checked at each cycle.
Outcome
Conception in the second IUI cycle. Total cost was a small fraction of a single IVF attempt.
The point of this case
Tablets prescribed without scan monitoring are guesswork. Once we could see a follicle growing and time the insemination to within a few hours, the odds changed completely.
