Eighteen months of trying with cycles varying between 35 and 60 days. She had been offered IVF at a consultation elsewhere without any investigation at all.

What the workup showed

TSH was 8.2 mIU/L with positive anti-TPO antibodies — subclinical hypothyroidism, untreated. Prolactin was mildly raised. AMH, tubes, uterus and her husband's semen analysis were all normal.
