What she came in with

She arrived with a folder from another centre: two full IVF cycles, both with a long agonist protocol, both cancelled or converted after producing only three and four eggs. She had been told her ovaries 'did not respond' and that donor eggs were the next step.

What the workup showed

AMH was 1.4 ng/ml with an antral follicle count of nine — reduced, but not the near-absent reserve that had been implied. Reviewing the old cycle sheets showed both attempts had used down-regulation, which suppresses an already modest reserve further before stimulation even begins.

What we did

An antagonist protocol with a lower starting dose and no down-regulation, with dual trigger. Seven eggs were collected, five were mature, four fertilised with ICSI and two reached blastocyst. Both were frozen and one transferred in a prepared cycle six weeks later.

Outcome

A positive beta hCG, a single intrauterine pregnancy on the six-week scan, and delivery at 38 weeks. One blastocyst remains in storage.
