What she came in with

Severe intrauterine adhesions following a postpartum curettage, with no menstruation for three years. They had been told by an agent that surrogacy could be arranged and had already paid a deposit.

What the workup showed

Hysteroscopy confirmed dense adhesions obliterating most of the cavity. The medical indication for surrogacy was genuine. The eligibility question, however, was separate — and their circumstances did not meet the Surrogacy (Regulation) Act's requirements on the surrogate they had identified.

What we did

We set out the Act's criteria in writing at the first consultation, referred them to legal counsel, and in parallel offered hysteroscopic adhesiolysis with a view to restoring the cavity, which had not been attempted.

Outcome

Two adhesiolysis procedures with oestrogen and PRP restored a functional cavity. She conceived through IVF and carried the pregnancy herself.
