ICSI for patients from New Friends Colony
New Friends Colony and Maharani Bagh are a straightforward run to the Greater Kailash OPD along Mathura Road.
Intracytoplasmic sperm injection is used when sperm cannot fertilise an egg on their own. Under a high-powered microscope, the embryologist selects one sperm and injects it directly into the egg's cytoplasm. Everything before and after is the same as a standard IVF cycle.
ICSI transformed the outlook for severe male factor infertility. It is also used after unexpected total fertilisation failure in a previous IVF cycle, and when only a small number of eggs are collected and every one matters.

Who we treat from New Friends Colony
- Very low sperm count, poor motility or abnormal morphology
- Azoospermia, where sperm are retrieved surgically by PESA or TESA
- Previous IVF cycle with complete or near-complete fertilisation failure
- Frozen-thawed or surgically retrieved sperm, and very small egg numbers
How treatment runs
Egg retrieval
Identical to a standard IVF cycle — stimulation, monitoring and a day procedure.
Sperm preparation
The sample is processed and the most structurally normal, motile sperm are identified.
Injection
Each mature egg is injected individually with a single selected sperm.
Culture and transfer
Fertilisation is checked at 18 hours, embryos are grown to blastocyst and one is transferred.

Case study: A normal female workup, and an unexamined male factor
31 and 36, Dwarka · two years trying · all female tests normal
What she came in with
Two years of investigation focused entirely on her. Hormones, tubes, uterus, thyroid — all normal, repeatedly. Her husband had been told a semen report was 'slightly low' and left at that.
What the workup showed
A repeat semen analysis to WHO criteria showed a concentration of 3 million/ml with 12% progressive motility and 1% normal forms. A DNA fragmentation index of 34% was high. A scrotal ultrasound found a grade 2 varicocele.
What we did
Three months of varicocele repair, antioxidants and lifestyle correction improved fragmentation to 19% but left the count severely reduced. An IVF cycle with ICSI followed, injecting each of the eleven mature eggs individually.
Outcome
Nine eggs fertilised, four reached blastocyst. A single transfer produced an ongoing pregnancy; three blastocysts remain frozen.
The point of this case
Two years and a great deal of money were spent investigating the partner who did not have the problem. A semen analysis costs very little and takes two days.
Illustrative case. This is a composite of presentations commonly seen in this practice, written for explanation — it is not the record of an identifiable patient, and no individual's details are used. Outcomes vary, and nothing here is a prediction of what will happen in your case.

Your reports get read before anything is sold to you
Most couples arrive with a folder of tests from elsewhere. We go through it first. In a surprising number of cases the next step is cheaper and simpler than what was recommended before.
One doctor, start to finish
Dr. Tiwari performs your scans, your retrieval and your transfer, and looks after the pregnancy afterwards. You are not handed between juniors at each visit.
Costs in writing, before you commit
An itemised estimate covering drugs, lab charges, anaesthesia and freezing. If a cycle needs to be abandoned mid-way, you are told what that changes financially on the same day.
