Case study: A normal female workup, and an unexamined male factor

What she came in with

Two years of investigation focused entirely on her. Hormones, tubes, uterus, thyroid — all normal, repeatedly. Her husband had been told a semen report was 'slightly low' and left at that.

What the workup showed

A repeat semen analysis to WHO criteria showed a concentration of 3 million/ml with 12% progressive motility and 1% normal forms. A DNA fragmentation index of 34% was high. A scrotal ultrasound found a grade 2 varicocele.

What we did

Three months of varicocele repair, antioxidants and lifestyle correction improved fragmentation to 19% but left the count severely reduced. An IVF cycle with ICSI followed, injecting each of the eleven mature eggs individually.

Outcome

Nine eggs fertilised, four reached blastocyst. A single transfer produced an ongoing pregnancy; three blastocysts remain frozen.
