What early pregnancy scan involves
An early pregnancy scan is usually done between six and nine weeks, by the transvaginal route because the abdominal view at this stage is poor. It answers four questions: is the pregnancy inside the uterus, is there a heartbeat, how many are there, and how many weeks are you actually at.
The dating matters more than most people realise. Every later decision in the pregnancy — when the NT scan is due, whether the baby is growing on track, whether induction is needed — is measured against this date. A due date set by a crown-rump length at eight weeks is accurate to within three or four days; one calculated from a remembered period date often is not.

What she came in with
A positive test with cycles that ranged from 32 to 45 days. Her calculated due date from the last menstrual period placed her at nine weeks and five days.
What the workup showed
The transvaginal scan showed a single intrauterine sac with a fetal pole and a clear heartbeat, but a crown-rump length corresponding to eight weeks and two days — ten days behind the dates. She had ovulated late, which long cycles make likely.
What we did
The due date was reset to the scan measurement, which is accurate to within three to five days at this gestation. All subsequent scan timings were recalculated from the corrected date.
Outcome
The NT scan was booked in the correct window, the anomaly scan was done at the true 19 weeks, and growth was plotted against the right gestation. She laboured spontaneously at what was genuinely 40 weeks.
The point of this case
A due date ten days out does not sound serious until the baby is called growth-restricted at a gestation it has not reached, or induction is advised a week and a half early.

Questions about early pregnancy scan
Is a transvaginal scan safe in early pregnancy?
Yes. It uses the same ultrasound energy as an abdominal scan and does not touch the pregnancy. It does not cause miscarriage or bleeding.
What if there is no heartbeat at six weeks?
It is often simply too early, particularly if your cycles are long. A repeat scan seven to ten days later usually settles it. We do not make a diagnosis of miscarriage on a single early scan.
Why is my due date different from the one I calculated?
Because ovulation does not always happen on day fourteen. The scan measurement is more reliable than the date arithmetic, and we go with the scan.
