Chemotherapy and pelvic radiotherapy can permanently damage ovarian reserve and sperm production. Preservation before treatment starts is often possible, and these referrals are treated as urgent — we will see you within a day or two.

What she came in with
Referred by her oncologist on a Friday with chemotherapy planned to start in under three weeks. She was mid-cycle, day 19, which under older protocols would have meant waiting a fortnight she did not have.
What the workup showed
Oestrogen-receptor-positive disease, so conventional stimulation raising oestradiol into the thousands was not appropriate. AMH was 3.1 ng/ml with a good antral follicle count.
What we did
A random-start letrozole-based protocol begun the following Monday, keeping peak oestradiol below 500 pmol/L, with an agonist trigger. Direct discussion with the oncologist at the outset to confirm the timeline.
Outcome
Fifteen mature eggs vitrified on day eleven. Chemotherapy started three days later, on the original schedule.
The point of this case
Random-start protocols removed the two-week wait, and a letrozole-based approach made stimulation appropriate in hormone-sensitive disease. The delay to cancer treatment was zero.
