Case study: Two problems solved by one device

What she came in with
Family complete after two children, heavy periods causing borderline anaemia, and a request for permanent contraception. Her husband was reluctant about vasectomy and she was unenthusiastic about surgery.
What the workup showed
Haemoglobin 10.4 g/dl. Ultrasound showed mild adenomyosis, no fibroids, and a normal cavity. Endometrial biopsy was benign. Her blood pressure and migraine history ruled out combined hormonal methods.
What we did
A levonorgestrel intrauterine system was discussed alongside every other option, including tubal ligation and vasectomy, with failure rates and reversibility for each. She chose the IUS. It was fitted in clinic in under ten minutes.
Outcome
Bleeding reduced to light spotting by four months and stopped entirely by eight. Haemoglobin normalised. Contraception is effective for eight years with nothing to remember.

Questions about contraception & family planning
Will the pill affect my future fertility?
No. Fertility returns to your baseline within a cycle or two of stopping. The injectable is the one exception, where return can take several months.
Is a copper IUD suitable if I have not had children?
Yes. Insertion may be slightly more uncomfortable, but it is safe and effective. A hormonal IUD is often preferred if periods are already heavy.
